Information For Referring Physicians

Referrals can now be completed and submitted online.
Please advise the preferred location for referral, wait time and services vary between locations.
Please note that referrals must also be faxed to our office fax # 647-494-8687.
Please use our referral form or you may use your own letterhead.
We will arrange for cardiac diagnostic testing for referred patients.
Patients can book appointments online.
Referring doctors will get an appointment notification via fax once the patient is informed of the appointment.
If you want an earlier appointment please contact us by fax and send us more medical information.
Our wait times for new elective consultation are less than 4 weeks.
