Additional Information

       1. Every patient requires a referral from his or her physician
          
If you think that you or child one may have a heart problem, bring this to the attention 

           of a physician or other primary care provider. That doctor is the best person to decide  

           whether a pediatric/ adult congenital cardiology consultation will be helpful for you and,           if so, will make the referral. Our office sees patients via appointment only. No walk-in 

          accepted. 


      2. What to expect?

  • Arrive 5 minutes before the appointment time in order to finalize all necessary paperwork.
  • New patient visit includes having a height, weight, blood pressure and sometimes an  electrocardiogram (ECG). 
  • A nurse/ clinical assist. may help during the clinic visit with some of the assessment. 
  • Cardiologist may also recommend further cardiac testing, if needed. 
  • Your visit can last up to 3 hours.


      3. Tips on Preparing Your Child
        
It is important that your child feels comfortable and safe when visiting us. General 

         strategies for preparing your child for their upcoming visit:

  • Communicate openly with your child and be honest about their situation and the reasons for their visit.
  • Reassure your child that having to go to the clinic does not mean that they have done something wrong
  • Encourage your child to bring a favorite toy or activity from home to their appointment. This will help them feel safer and more comfortable at their appointment.


     4. Free Parking is available on site. 


     5. Please bring valid OHIP card and list of medications to the visit


     6. Rescheduling the appointment  
         If you are unable to keep your appointment, 48 hours notice is required to reschedule   

         your appointment; or else it may be considered a no-show. 


     7. No show or missed appointment
         If you miss the appointment we may not be able to make an appointment without a new 

         referral. 


     8. For release of medical records, we require the release of health information form. It can 

         be downloaded below. In some cases administrative fees may apply.
